Active for health
Exercise can help to manage, improve or reduce the risk of developing many health conditions.
At Two Riversmeet Leisure Complex, we run an Active 4 Health exercise referral programme that lasts for 12 weeks.
If you have a condition that affects your health and wellbeing, you could be referred to us by a health professional (i.e. GP, nurse, physio). If you are unsure, please do not hesitate to contact the Lifestyle Co-ordinator for more information.
After your programme, you will continue to be supported and guided to suitable opportunities to help you maintain your new lifestyle.
You can qualify for an Exercise Referral if you have 1 of these health conditions:
- Arthritis
- Asthma or COPD
- Back Pain
- Cancer (pre, during or post treatment)
- Chronic fatigue and ME
- Coronary artery disease or peripheral vascular disease
- Dementia or Alzheimer's
- Diabetes (Type 1 or Type 2)
- Fibromyalgia and polymyalgia
- General mobility issues
- High blood pressure
- High cholesterol
- Mental ill health (i.e. stress, depression, anxiety)
- Multiple Sclerosis
- Osteoporosis
- Overweight/obesity (BMI>40 kg/m2)
- Parkinson's disease
- Post-surgery
- Previous heart surgery / heart attack (must have completed phase 3 cardiac rehab)
- Risk of falls / balance issues
- Sedentary lifestyle
- Stroke or brain injury
You can qualify for an Exercise Referral if you have 3 of these health conditions:
- Smoker
- Prediabetes
- Family history of heart disease
- Overweight with a BMI of less than 30
- High Cholesterol 5.2mmol/L+
- Sedentary lifestyle
- A blood pressure reading of 140/90 or higher
If you meet this criteria then you can ask your local health professional to complete an: Active 4 Health Exercise Referral Form.
Your form is then sent to the Lifestyle Co-ordinator who will contact you to help you get started on one of our two pathways - either a personalised exercise programme or entry level activities.
A personalised exercise programme
This would be created to suit your health needs and interests. You will have one to one consultation/s with a specialist instructor and opportunities to exercise with others in weekly classes, both indoors and out.
Entry level activities
If it is more appropriate for your health needs and choice, you will be fully supported into a range of entry level activities rather than the more structured programme.
Cost
Your programme will last for 12 weeks. The initial consultation will be £18.40
If you pay as you go a gym session, swim session or group exercise class is £4.65 per session
Memberships are £27.50 per month by direct debit or £165 up front for a 6 month membership
