This programme is designed for anyone who needs extra support to become more active. You’ll receive a personalised plan and ongoing guidance to help you build confidence and maintain your new lifestyle. After completing your 12-week programme, we’ll continue to support you with suitable activities so you can keep moving forward.
GP referral

You may qualify if you have one of these conditions:
- Arthritis
- Asthma or COPD
- Back pain
- Cancer (pre, during or post treatment)
- Chronic fatigue or ME
- Coronary artery or peripheral vascular disease
- Dementia or Alzheimer’s
- Diabetes (Type 1 or Type 2)
- Fibromyalgia or polymyalgia
- Mobility issues
- High blood pressure or cholesterol
- Mental health conditions (stress, depression, anxiety)
- Multiple Sclerosis
- Osteoporosis
- Obesity (BMI > 40)
- Parkinson’s disease
- Post-surgery or previous heart surgery/heart attack (after phase 3 cardiac rehab)
- Risk of falls or balance issues
- Sedentary lifestyle
- Stroke or brain injury
Or if you have three of these risk factors:
- Smoker
- Prediabetes
- Family history of heart disease
- Overweight (BMI < 30)
- High cholesterol (5.2mmol/L+)
- Sedentary lifestyle
- Blood pressure 140/90 or higher
Ask your GP to complete an Active 4 Health Exercise Referral Form. Once we receive it, our Lifestyle Co-ordinator will contact you to discuss your options: a personalised exercise programme or entry-level activities for a gentler start. Your programme lasts 12 weeks and begins with an initial consultation (£18.40). Pay-as-you-go sessions: £4.65 per gym, swim or class. Membership options: £27.50 per month (Direct Debit) or £165 for 6 months upfront.
Gentle circuits: a whole-body exercises at your own pace.
Health circuits: ideal for those returning to exercise.
COPD class: gentle circuits focusing on breathing and endurance.
Back care: strengthen your back and core.
Chair fit: improve strength and mobility with seated exercises.
